Please complete all forms after selecting which athletic activities your student is interested in participating in for this upcoming school year. The IHSAA and Andrean High School require an updated IHSAA physical for the 24-25 school year to be completed AFTER 4/1/2024. We do not accept any other physical forms. Physicals must be completed in Indiana by an Indiana doctor. 


Incomplete forms and/or a lack of a new physical will result in your student-athlete not being eligible to participate.
