Please complete all forms after selecting which athletic activities your student is interested in participating in for this upcoming school year. The IHSAA and Andrean High School require an updated IHSAA physical for the 23-24 school year to be completed AFTER 4/1/2023. We do not accept any other physical forms.


Incomplete forms and/or a lack of a new physical will result in your student-athlete not being eligible to participate.
